What Are The Treatment Options For Tight Foreskin?

A tight foreskin should always be handled by a doctor or nurse who knows what they are doing. Phimosis is the medical term for a foreskin that can’t be pulled back. Paraphimosis is the medical term for a foreskin that has been pulled back but cannot be put back.

Physiological phimosis is something that boys are born with. At first, their skin stays in place on its own. As the natural bonds between the inner surface of the foreskin and the head of the penis break down over time, boys learn to pull the foreskin back and push it forward as they explore their genitalia. Most boys can do this by the age of 7, but some need more time.

Pathologic phimosis can happen in boys as young as 13 and in men of any age. It can be caused by a naturally tight ring at the tip of the foreskin, bad cleanliness, an infection, or inflammation. Sometimes, paraphimosis happens. This is like phimosis, except that once the foreskin has been pulled back, it can’t be pulled forward again.

Treatment Options

A doctor might give you a steroid cream to treat phimosis. This cream softens the foreskin, making it easy to move back and forth. Most of the time, the cream is put on the tip of the foreskin. After a couple of weeks, you may be able to do some gentle stretching movements. Patients (or their parents or guardians) should be shown how to do these movements correctly.

In more serious cases of phimosis, a surgery called circumcision may be needed to remove part or all of the foreskin. If only part of the foreskin is taken off, phimosis could come back.

Preputioplasty, or dorsal slit, which involves making an incision to widen the foreskin and loosen it, is another surgical choice. Nothing is taken away from the foreskin, but it should be easier to pull back and pull forward.

Paraphimosis

When the foreskin can’t be pulled forward from a retracted position, which is the case with paraphimosis, the condition might be handled by hand. Most of the time, a local anesthetic is used to numb the area before the doctor pulls the foreskin forward with his or her hands. (Note: Only a trained health care worker should do this.) If the paraphimosis is bad, the person may need surgery.

Paraphimosis is a medical emergency that needs to be taken care of right away so that more problems, like tissue death, don’t happen.

Summary

Don’t try to pull the foreskin back by force. This can hurt a lot. It can also cause small cuts, which can leave scars that make it harder to pull the foreskin back. This is called acquired phimosis or phimosis caused by a disease.

If the foreskin is tight, it could also get stuck behind the head of the penis (glans) when it is pulled back. This could make a ring that cuts off blood flow to the glans. It’s called paraphimosis. It is a medical issue that needs to be taken care of right away by a doctor.

In the first few years of life, you don’t have to pull the foreskin back all the way to clean the penis. It only needs to be washed on the outside. It’s not a good idea to try to clean the space between the foreskin and the head of the penis with something like a cotton swab because that could hurt or damage the sensitive skin there.
